Fixed computation of SSIM in GlStructuralSimilarity processor. Extended SsimDemo pipeline to compute batch-wise SSIM of an entire US sequence.

Added a GlStructuralSimilarity processor to compute the SSIM of two 2D images. This processor is not thoroughly tested yet.

Added a field _pipelineName to AutoEvaluationPipeline. This allows to implement AutoEvaluationPipeline::getName() so that AutoEvaluationPipeline is no longer abstract. Consequently, the LuaPipeline is no longer needed.

This change was also used to change the AbstractPipeline constructor signature to pass-by-reference instead of pass-by-pointer for the pipeline's DataContainer. This presents the semantics more clearly since the DataContainer must not be 0.

Introducing workflowdemo module showing a demo implementation on how to use the CAMPVis workflow interface.

Hence, all this enables the first usable implementation of a specific pipeline:
The SliceVis pipeline combines MhdImageReader and SliceExtractor for a very simple 2D slice rendering
